An evaluation report is strongest when it brings together everything you gathered: your testing and observations, what caregivers and teachers reported, and standardized scores. Report Wizard combines all of it into one report, so there is no copying and pasting between documents.
What you can include
- Guided evaluations completed in MyTherapyWizard (required for a full evaluation report)
- Questionnaires returned by caregivers, teachers or the client, such as the FPOM or ENGAGE-S
- Assessments: standardized scores you entered, such as the Beery VMI or PDMS-3. The report lists the test name, description, date given and scores.
- Appointments and session notes, when relevant
Steps
- Open Report Wizard and choose the client and dates.
- Select Full Evaluation Report (or Abbreviated Evaluation) and your target audience.
- On the data screen, select the evaluation, then check the questionnaires and assessments to merge in.
- Add optional context for any section, then generate.
- Review, edit and e-sign.

Built on structured, scored results
MyTherapyWizard is an EMR with clinical protocols built in:
- Guided Evaluations are scored with structured scoring: the same criteria, rules and performance bands for every client.
- The plan of care is generated from that evaluation, with goals linked to the baseline.
- Once the plan of care is active, its goals populate into your session notes. Each goal gets a Goal Performance Index (GPI) that combines accuracy with the support needed, so 70% independent and 70% with maximum assistance score differently. Each session gets a Session Performance Index (SPI), the average of its GPIs.
- Reports draw from these scored results plus the comments and narratives you wrote. AI only helps with wording. It never changes a score, and you approve every line.
