Writing Functional, Measurable IEP Goals With the Goal Bank

Writing Functional, Measurable IEP Goals With the Goal Bank (MyTherapyWizard help article card)

One in five concern posts in a large FB OT group asks some version of "what should I work on?" Many are about goals: impairment goals that loop forever, goals inherited from another district, or sensory and executive function goals that are hard to measure.

✨ Available on: All plans

🧩 A functional goal names five things

  • The student and the observable action
  • The school routine or context where it happens
  • The support the student needs
  • The success criterion
  • How it will be measured

Example: "During morning arrival, the student will unpack and hang up her backpack with no more than one verbal cue on 4 of 5 days, measured by weekly teacher checklist."

🪄 In MyTherapyWizard

  1. Open the Plan of Care and select Goals.
  2. Choose From Bank, or build one with the sentence builder: verb, descriptor, qualifier, core skill, context and success definition.
  3. Set measurement type, support level, mastery rule and baseline.
  4. Or select Generate Goals. Suggestions follow developmental sequencing from the evaluation, and you can edit or override any of them.
  5. Save your best goals to Team favorites so new staff start from them.

💡 Why it matters: goals are stored as parts, not sentences, so every session note can measure against the same baseline and target.

Built on structured, scored results

MyTherapyWizard is an EMR with clinical protocols built in:

  • Guided Evaluations are scored with structured scoring: the same criteria, rules and performance bands for every client.
  • The plan of care is generated from that evaluation, with goals linked to the baseline.
  • Once the plan of care is active, its goals populate into your session notes. Each goal gets a Goal Performance Index (GPI) that combines accuracy with the support needed, so 70% independent and 70% with maximum assistance score differently. Each session gets a Session Performance Index (SPI), the average of its GPIs.
  • Reports draw from these scored results plus the comments and narratives you wrote. AI only helps with wording. It never changes a score, and you approve every line.
