Code changes are the single biggest topic among practice owners, led by the proposed replacement of CPT 92507 with new timed speech-language codes for 2027. Final rules come from CMS and the AMA, and Medicaid and commercial payers decide separately when and how they adopt them. Watch your state association and payer bulletins for effective dates.

🧩 When a code changes
- Read the payer's effective date and any new unit or timing rules.
- In Payer & Codes › Procedure Codes, add the new codes, or select Sync from ClaimMD.
- Enter the allowed amount for each payer in Rates. Rates carry effective dates and can be end-dated, so the old rate stays on record for earlier dates of service while the new rate applies going forward.
- Update Bundles that included the old code, and check Modifiers.
- Adjust Insurance limits (units per day, units per plan of care) in Flow Settings if the new codes are timed.
- After the effective date, watch Needs Action for signed notes missing a procedure code.
💡 Why it matters: codes, rates and limits live in one place, so a code change is a settings update, not a scramble across spreadsheets.
